Due to changes in policy at the Board of Regents and Institutions in the University System of Georgia, teams will be required to complete the following forms. Eachstudent attending hasto complete each form (5 forms per student).
Consent and Release for Minor’s Presence in Laboratory Form = This form must be completed by the student if 18 years of age or older or a parent / legal guardian.
Rules for Minors and Visitors in Research Laboratories and Vivarium Facilities Form = This form must be completed by the student if 18 years of age or older or a parent / legal guardian.
Medical form = This form must be completed by the student if 18 years of age or older or a parent / legal guardian and also signed by the team coach.
Pickup Authorization = This form must be completed by the student if 18 years of age or older or a parent / legal guardian and also signed by the team coach.
Agreement Waiver = This form must be completed by the student if 18 years of age or older or a parent / legal guardian and also signed by the team coach.
The five forms above must be collected by the team coaches. They need to be scanned in and saved as pdf files.
Team coaches will email five separate pdf files per team to the State Director for the Science Olympiad:
- One file contains all of the consent and release forms for all team members.
- One file contains all of the Rules for minors and visitors for all team members.
- One file contains all of the medical forms for all team members.
- One file contains all of the pickup authorizations for all team members.
- One file contains all of the agreement waivers for all team members.
Each school will also need to complete the team roster form (excel file located here).
Upload the forms per team on the website ( https://statescioly.org/ga/Account/Login ) as Head Coach Documents. You should be uploading 5 files per team. Uploading to the website is the preferred method. File name should include your school name, which team it is for if you have multiple teams, and which form set it is. Example: Great Fall HS, Team 1, Consent.pdf. Email the forms to georgiascioly@gmail.com as a last resort.
Any changes to teams after the above forms have been submitted are allowed. GSU requires us to send these forms to the administration prior to the event and so having them as complete as possible is appreciated. If changes do occur, you will need to email the scanned forms to the State Director for the Science Olympiad Electronic copies of the forms are essential.
